i filled it as well, but it's somehow faulty at part where you need to p.ex. specify addons and so on...

Done and filled out. I believe Texmod counts as an "addon" as it can modify certain aspects of the game to be different visually, such as health bars, maps, etc. I count those as addons.

you might want to go check out Second Life, its a whole mmorpg that revolves around add ons.

Just to help you out, your survey is fatally flawed.
You have only yes or no for all questions in which someone may have never played the game. Therefore, you will fundamentally get a much larger percentage of NO answers when reviewing the compiled data, and will have a indeterminately skewed spread sheet of answers. I suggest starting over and adding a N/A option to any game-specific questions.

As empirical as he desires in order to get an accurate study of his subject. Regardless of your perception of the study of games, I would prefer to be as accurate as possible when it comes to doing any sort of research.
The flaw isn't a minor one. If only makes sense if you have played all three games he proposes. If you haven't then the answers get skewed and if it is for school and he has to explain it, then hes SOL. No professor of any acedemic regard would award a good grade based on a flawed survey.
